If you have any questions or concerns about your insurance coverage, please don't hesitate to contact our office. To ensure a seamless insurance verification process, we kindly request that you bring a copy of your insurance card to your first appointment or send it to us via secure fax or mail prior to your visit. In the event that we are unable to verify your insurance information at the time of your appointment, you may be considered a self-pay patient until we are able to obtain the necessary information. We thank you for your cooperation and understanding.
At our medical clinic, we are pleased to accept a wide range of insurance plans and we are most plans, including most versions of popular providers such as: Accountable PPO, Affiliated PPO, Aetna, BCBS, Beech Street PPO, Cigna, CCN PPO, Galaxy Healthcare PPO, Great West Healthcare HMO and PPO, Humana/Choice Care PPO, Health EZ, Medicare, MultiPlan, Pacificare PPO, PHCS, PPO, Unicare Classic/Performance, and United Healthcare. It is important to note that this list is not exhaustive and may be subject to change.
Unfortunately at this time we are NOT taking any Baylor Scott & White Plans, Aetna Medicare Advantage (we are on all other Aetna plans), Cigna Local Plus & Cigna HealthSprings (but we are on most other versions of Cigna), Molina or Medicaid at this time.
We do accept self pay and will offer a discount for services when not insured.
For the most up-to-date information on the insurance plans we accept, we recommend contacting our office. To ensure a smooth visit, we also advise all new patients to obtain necessary authorizations from their insurance company prior to their appointment at our clinic.
